Terms and Conditions

These terms and conditions are entered into by and between 15608741 Canada Inc. operating as District Fitness (hereinafter: District) and You (hereinafter: the Member).
Services: The District membership entitles the usage of the District Fitness Facility and the Gear Up mobile app.
Payment: The Member commits to paying the membership fee on a recurring, monthly basis until the Member cancels. Membership fee will be automatically deducted each month between the 1st and the 5th.
Cancellation: The Member has the right to cancel their membership at any time by sending a request to info@district-fitness.ca. Member will be charged one more monthly membership fee.
By electronically signing this contract, I acknowledge my enrollment and participation in fitness activities at District Fitness. I understand the potential risks and hazards associated with physical exercise. I understand that it is my responsibility to consult with a healthcare professional before participating in any exercise program, and I affirm that I have either:
• Been given permission by a licensed physician to participate in exercise, or
• Determined that I am in good health and have no medical conditions that would prevent me from safely participating in exercise.
I understand that I am solely responsible for my own health and well-being while using the facilities and participating in activities at District Fitness. I agree to follow all rules and guidelines provided by the fitness center staff and to use the equipment and facilities in a safe and responsible manner.
I further acknowledge and understand that District Fitness, its staff, and instructors are not licensed medical professionals and do not provide medical advice, diagnosis, or treatment. I will not hold them responsible for any injuries or medical conditions that may arise during my participation in fitness activities.
I hereby release, waive, and discharge District Fitness, its owners, employees, and representatives from any and all claims, liabilities, or demands, including but not limited to personal injury, sickness, or death, whether caused by negligence or otherwise, arising from or in any way connected with my participation in fitness activities at District Fitness.
I have read this Health Waiver and understand its contents. By enrolling and participating in fitness activities at District Fitness, I voluntarily agree to the terms and conditions outlined in this waiver.
